Arkansas Child Health Advisory Committee (CHAC).  Created through ACT 1230 in 2003, the CHAC was created to address childhood obesity and develop statewide standards for physical activity and nutrition.

California Department of Health Services

Colorado Department of Public Health and Environment, School Site Resource Tool Kit, Implementation Guide for the Colorado Physical Activity and Nutrition State Plan 2010Tool to assist school personnel to implement strategies to promote healthy nutrition and physical activity in schools.

New Hampshire Department of Health and Human Services,

Foundation for Healthy Communities: Community Health Initiative.  The FHC has initiatives in several cities, including Dover, Rochester, Laconia, Keene, Franklin, Manchester and Nashua focusing on promoting cardiovascular health through targeted activities that focus on physical activity and cholesterol awareness. FHC is a non-profit corporation that exists to improve health and health care in New Hampshire and includes providers, hospitals, health plans, consumer, and policy leaders.
