National EMSC Resource Center, jointly sponsored by the US Departments of Health and Human Services, Health Resources and Services Administration (HRSA) and Department of Transportation, National Highway Traffic Safety Administration (NHTSA). Emergency Information Form for Children with Special Care Needs, American Academy of Pediatrics. Site provides a 2-page form for emergency providers and caregivers that has important information in case of emergency for children with special health care needs.
