Emergency Information Form for Children with Special Care Needs, American Academy of Pediatrics. Site provides a 2-page form for emergency providers and caregivers that has important information in case of emergency for children with special health care needs. Developed by the American Academy of Pediatrics and the American College of Emergency Physicians (pdf format).
